Healthcare practitioners who authorize the medical use of cannabis must use the updated version of the Medical Cannabis Authorization Form beginning Sept. 1. 

The revised form reflects changes made last year, including the removal of the requirement to identify a patient’s specific qualifying condition and an acknowledgment by the healthcare practitioner that the patient’s first office visit must be conducted in person.

Practitioners are encouraged to begin using the updated form as soon as possible. If you still have tamper-resistant paper printed with the previous version of the form, you may continue using it until the Sept. 1 deadline.
